Cancellation, No Show, Late Policy 

Dear Patients, 

Integrity Medical Center and Integrity Natural Health have instituted a policy for Appointment Cancellation/Rescheduling, No Show, and Late Arrival.  

• Cancellations need to be made at least 24 hours in advance of the appointment to avoid incurring a $50 late fee. This will allow us to make the appointment time available to other patients. This also applies to rescheduling appointments. 

• If you are more than 15 minutes late, your appointment may be cancelled and rescheduled so that your time with the provider will not be limited or will not infringe on other scheduled appointments. 

• Repeated missed appointments may result in termination of the provider/patient relationship.
